How we are Run

Crossroads Care - Forest of Dean & Herefordshire is a registered charity (Charity Number 1086841) and a company limited by guarantee. Company Number 4178148 We have a board of voluntary trustees and are a member of the Crossroads Association which is the largest voluntary organisation providing support to carers in their own homes with over 150 schemes across the country.

We are staffed by a team of Care managers
and Care co-ordinators headed by the Chief Executive.

Our quality Standards

Crossroads Care - Forest of Dean & Herefordshire is registered and regulated by the Commission for Social care inspection and rated as an EXCELLENT service. The inspection report can be viewed and downloaded on the CSCI website.

Our Carer Support Workers are trained and supervised in line with National Minimum standards and receive additional training in subjects such as Stroke awareness and Palliative Care.

In addition we are audited by the National crossroads scheme and have achieved CROQUET level 2 status.

When we operate

Our service is operational every day of the year.

Our office is manned from 08.30-16.30 Mon-Fri.

Outside office hours there is an answerphone facility for non urgent calls and an out of hours emergency number (07988 996855) for more urgent calls,

Process for the delivery of care and support

Crossroads receives referrals from a variety of sources including self referrals, family and friends, District Nurses, Adult and Social Care Services, Primary Care Trusts and other organisations.

When a referral has been made a member of the Crossroads team will arrange to visit to discuss the range of services offered.

We will then agree your care plan with you. In order to maintain your safety and the safety of our staff the agreed tasks will be risk assessed, You will be given information regarding our service which will include how to make complaints or give complements.

We will review your care package within 6 weeks and then every 6 months. You  can of course request more frequent reviews if you feel your needs have changed.

How Services are funded

You may have your service funded by Adult and Social Care Services, The Council or the PCT.

You may be in receipt of Direct Payments or choose to fund your own care.

However your service is funded we will assure you of a high quality professional service at all times.
